Please remember that it is your responsibility to bring a referral to your appointment if your insurance requires one to see our specialists. Co-payments are expected at the time of service.  If for any reason you feel you will not be able to make your payments at the time services are rendered, please call our office prior to your appointment and speak with someone about financial arrangements. 

If you are unsure if our providers are covered under your insurance plan, please call your insurance company and check prior to your appointment to prevent any non-covered services, or any out of network charges.

Currently, our physicians are NON-PARTICIPATING with the following:

• MEDICARE CHOICE

• AVMED 

• QUALITY HEALTH CARE PLAN (Medicare HMO based program)
